What we collect

When you reserve a program: your email, name, ZIP code, and if you choose, your mobile number, plus your confirmation that you are 18 or older. When you check your coverage: your email, ZIP code and the name of your health plan. Our servers also see basic request data (such as your IP address and country) to block spam; we store only a one-way scrambled version of your IP address.

Site usage

We count anonymous site events, such as page views, button clicks, and whether a quiz was started or finished, to improve the site. These use a random number that lasts only until you close the tab. They include no cookies, no name or email, and none of your quiz answers. Cloudflare Web Analytics also counts page views without cookies.

What we do not collect

The strength quiz runs entirely in your browser. Your quiz answers, including anything about your health or nicotine use, are never sent to us or stored by us. We do not take card details on this site, use advertising trackers, or sell personal information.

How we use it

To confirm your order, answer your coverage question, and send program updates. You can unsubscribe from any email.

Sharing

We use service providers to run this site and send email (for example, our hosting provider, Cloudflare). They process data only on our behalf. We do not sell or rent your information.

Your choices

To see, correct, or delete your information, email privacy@katzmintz.com and we will respond within 30 days.

Health information

Before we collect any health information (for example, for an insurance clinician review), we will ask for your consent and publish a consumer health data privacy policy on this page.
